VISA FORMALITIES

All the participants and accompanying persons are requested to send to local organizer, Ms. Olga Rein, their personal information (Personal Data for the Invitation Letter) for arranging the official invitation approved by the Ministry of Foreign Affairs of the Russian Federation before May 18, 2003.
This includes: (1) Full name, (2) Sex, (3) Date of birth, (4) Country and place of birth, (5) Present nationality, (6) Original nationality, (7) Affiliation, (8) Position, (9) Official address (Post Zip, country, city, street, house No., fax No. - mandatory!), (10) Passport number, issue date and expiration date, (11) Period of stay in Russian Federation (from ... to ...), (12) Visited cities (St. Petersburg, Moscow, etc.), (13) The city where you are planning to apply for visa (city in your country where the Russian Consulate is located), (14) The hotel, where you are planning to live.

In addition, a copy of your passport first page will be required (sent by fax
+7 095 362-0373 or as e-mail attachment to star@ciam.ru).
